    What happened

Roche announced on September 30, 2026 (Basel time) that the U.S. Food and Drug Administration has accepted its New Drug Application for fenebrutinib under priority review, covering both relapsing multiple sclerosis (RMS) and primary progressive multiple sclerosis (PPMS).

Fenebrutinib is an investigational non-covalent BTK (Bruton tyrosine kinase) inhibitor. If approved, it would be the first BTK inhibitor and the first high-efficacy oral treatment for both RMS and PPMS. The filing is supported by three Phase III trials:

    • FENhance 1 & 2 (RMS) – fenebrutinib cut annualized relapse rates by 51% and 58% versus teriflunomide.
    • FENtrepid (PPMS) – a consistent positive trend in delaying disability progression, with numerical benefit versus Ocrevus, making it the first investigational medicine in more than a decade to slow disability progression in a Phase III PPMS trial.
    • Mechanism – designed to address both the acute inflammation that causes relapses and the chronic brain inflammation that drives disability progression.

For context, nearly 1 million Americans live with multiple sclerosis, a market where treatment is still dominated by infused or injectable therapies.

    Why it matters

A first-in-class oral BTK inhibitor would materially broaden Roche’s neuroscience franchise beyond Ocrevus, which has already been used to treat more than 525,000 people. An oral option with high efficacy could expand the treatable population and reinforce Roche’s pricing power as infusion-based franchises face biosimilar pressure.

The bigger prize is PPMS. Progressive MS has almost no effective treatment options, so fenebrutinib would enter a wide-open segment where even a modest disability-delaying benefit can drive rapid, premium uptake. A PPMS win would also re-rate the entire BTK inhibitor class and hand investors a clear read-through for biotech names with neuroinflammation or BTK programs in MS.

    What to watch

Priority review sets a target FDA decision roughly six months from filing, so the calendar catalyst is well defined. Investors should watch for full Phase III data presentations at upcoming neurology congresses, which will let physicians stress-test the disability-progression signal against Ocrevus more rigorously. Competitor readouts in the MS pipeline, especially other BTK inhibitors, are the other key catalyst to track.

How do you think a first oral BTK inhibitor could change the way MS is treated – would you expect neurologists to switch stable patients off infused therapies?
