The FDA has approved lirafugratinib, to be marketed as Lyrfigtu by Elevar Therapeutics, for adults with previously treated unresectable, locally advanced or metastatic cholangiocarcinoma (bile duct cancer) harboring an FGFR2 gene fusion or other rearrangement, according to reporting from Medscape and MedicalDaily on September 24–25, 2026.

    **What happened**
Lyrfigtu is the third FGFR2 inhibitor to reach the U.S. market, joining pemigatinib and futibatinib — but it is the first that selectively targets FGFR2 rather than broadly inhibiting FGFR1/3/4, a design that translated into lower rates of hyperphosphatemia (20.7%) and diarrhea (21.6%) in testing. Approval was based on the single-arm phase 1/2 REFOCUS trial in 116 patients new to FGFR2 blockers who had received prior chemotherapy or chemoimmunotherapy: objective response rate 46%, median duration of response 11.8 months, median progression-free survival 11.3 months, and median overall survival 22.8 months. The application received priority review plus breakthrough therapy and orphan drug designations. On safety, almost a third of patients had serious adverse reactions, including infection (6%), pneumonia (3.4%), fatigue (2.6%) and hemorrhage (2.6%), with one fatal hemorrhage reported.

    **Why it matters**
Selective FGFR2 inhibition with a differentiated tolerability profile gives Elevar a credible angle in a small but high-value oncology niche, where hyperphosphatemia management has historically complicated the class. A 46% response rate with nearly a year of durability in pretreated patients is a solid clinical package that should support pricing power and formulary placement. For investors, this continues the streak of targeted-oncology FDA wins this year — and with an orphan designation, Lyrfigtu enters a market with limited competition and high unmet need.

    **What to watch**
Watch for Elevar’s commercial launch timeline and initial uptake commentary, any confirmatory-study or label-expansion moves, and how physicians position Lyrfigtu against pemigatinib and futibatinib in treatment sequencing. Longer-term, read-throughs to other FGFR-driven programs and potential partnership or M&A interest around Elevar will be worth tracking.

Discussion: How quickly do you expect physicians to switch to a selective FGFR2 inhibitor like Lyrfigtu — and does this make Elevar a takeover target?
