    What happened

Shares of Capricor Therapeutics (CAPR) surged about 20% in after-hours trading Tuesday after new 24-month HOPE-3 extension data for deramiocel, its investigational cell therapy for Duchenne muscular dystrophy. The results were presented as a late-breaking poster at the 31st Annual Congress of the World Muscle Society in Hiroshima, Japan (Sept 29–Oct 3).

Patients who switched from placebo to deramiocel saw upper-limb decline slow by 76% — estimated decline on the Performance of Upper Limb 2.0 scale fell from 2.05 points to 0.49 points — while elbow-related function decline slowed by 66% (from 1.33 points to 0.45 points).

    Key figures:

    • Across the full HOPE-3 dataset, deramiocel slowed deterioration in upper-limb function by roughly 54% compared with placebo
    • Over two years, both groups lost less function than an external disease-progression model predicted
    • The drug was generally safe and well tolerated over 24 months
    • Regular-session close Tuesday: $8.57 (-2%), bringing the monthly decline to about 14%; the stock is up roughly 144% over the past month on anticipation
    • Short interest is about 38.2% of float (~22.2M shares)

    Why it matters

This is a major single-stock move on genuinely new trial data, landing about seven weeks before the FDA’s November 22, 2026 target decision date (PDUFA) for deramiocel’s biologics license application — refined around an upper-limb-function indication. The extension data is part of the submission under review and strengthens the upper-limb label case.

With nearly 40% of the float short, the setup into the PDUFA date is high-torque both ways. It follows a familiar biotech pattern: Iovance (IOVA) soared 31% on raised 2026 revenue guidance, and the FDA pipeline stays in focus ahead of tomorrow’s Camzyos decision for adolescents on Sept. 30.

    What to watch

The Nov. 22 PDUFA decision; any additional FDA commentary on the refined upper-limb indication; whether Wednesday’s session holds the after-hours gains.

After a 144% run plus tonight’s extension data, is the market already pricing in an approval — or is the risk/reward still asymmetric into Nov. 22? Reply below.
