Zealand Pharma just fired the starting gun on one of the most important obesity-drug trials of the decade — with Roche as its partner.
WHAT HAPPENED
On September 22, 2026, Zealand Pharma (Nasdaq: ZEAL) announced the initiation of the global registrational Phase 3a ZUPREME program for petrelintide, its once-weekly amylin analog for chronic weight management, conducted with partner Roche. The program comprises three trials — ZUPREME-3, ZUPREME-4 and ZUPREME-5 — enrolling approximately 7,000 participants in total: people with obesity or overweight without type 2 diabetes, with type 2 diabetes, and with established cardiovascular disease, respectively. The program builds on positive Phase 2 data showing up to 10.7% mean weight loss at 42 weeks versus 1.7% for placebo, with a tolerability profile largely comparable to placebo.WHY IT MATTERS
Petrelintide mimics amylin, which regulates appetite and gastric emptying — a different mechanism from the GLP-1 drugs that dominate the obesity market through Novo Nordisk and Eli Lilly. The March 2025 Roche–Zealand deal, worth up to $5.3 billion total with $1.65 billion upfront, shows Roche is betting big on amylin-based therapies as an alternative or complement to GLP-1s. Better tolerability could be the real prize: adherence is the key weakness of current obesity drugs, and a drug patients can actually stay on wins the long game.WHAT TO WATCH
Key catalysts include interim readouts from the three ZUPREME trials, any Roche commentary on commercial timelines, and competitive responses from Novo Nordisk and Eli Lilly as amylin-based challengers advance. Safety and tolerability data at Phase 3 scale will be the make-or-break datapoint for the whole amylin thesis.Related reading: FDA Approves Merck Welireg + Lenvima Combo for Kidney Cancer — https://bullounge.com/forums/topic/%e2%9a%a1-fda-approves-merck-welireg-lenvima-combo-for-kidney-cancer/
