Eli Lilly’s experimental combination drug EloraTZP beat Zepbound head-to-head in a Phase 2b trial, with the highest dose delivering 23.3% average weight loss over 48 weeks in patients with type 2 diabetes — well above the 14.8% seen on Zepbound alone. The data, presented at the EASD meeting in Milan on Wednesday, propels the tirzepatide-plus-amylin combo into Phase 3. Lilly plans to start Phase 3 trials in Q4.

    What happened

    Key figures:

    • 23.3% average weight loss on the highest EloraTZP dose vs 14.8% on Zepbound alone, over 48 weeks.
    • A1C fell up to 2.9% vs 2.4% for tirzepatide alone.
    • 367 adults with obesity or overweight plus type 2 diabetes enrolled.
    • Discontinuations ran 10.8%–27% on the combo vs 2.9% on Zepbound alone, driven by GI side effects.
    • Ken Custer, president of Lilly Cardiometabolic Health, said the combo could offer the best balance of efficacy, safety and tolerability in Lilly’s obesity portfolio.

    Why it matters

Winning head-to-head against your own blockbuster is the strongest possible signal. Zepbound is Lilly’s crown jewel; beating it by 8.5 percentage points in diabetic patients — a population where weight loss is typically harder — resets expectations for the entire next generation of obesity drugs.

The side-effect trade-off is the real story. Discontinuation rates up to 27% suggest tolerability, not efficacy, will decide the commercial ceiling. Lilly’s bet is that titration and dosing refinements in Phase 3 will close that gap — investors should take that promise with caution.

This also raises the bar in the Lilly–Novo Nordisk arms race. With retatrutide’s 28.3% benchmark (in non-diabetic patients, over 80 weeks) already on the board, combination therapy is clearly where the industry’s big money is going.

    What to watch

Phase 3 launches in Q4 — watch the trial design for how Lilly handles the GI tolerability problem. Any dosing data that narrows the discontinuation gap would be a strong positive read-through for the stock.

With discontinuations running as high as 27%, would you back EloraTZP to become Lilly’s next blockbuster?
