Novo Nordisk (NVO) has launched Wegovy HD, its new 7.2 mg once-weekly semaglutide injection, nationwide across the United States as of September 27, 2026 — making the highest-dose formulation of the blockbuster weight-loss drug available to patients.

    What happened

The US Food and Drug Administration approved Wegovy HD in March 2026 under the agency’s Commissioner’s National Priority Voucher program — the fourth approval under that accelerated review pathway. Wegovy HD is the first GLP-1 obesity medicine to show superiority over the standard 2.4 mg Wegovy dose in a trial. Availability nationwide began this week, expanding access to the strongest GLP-1 option on the market.

    Key figures

    • 7.2 mg once-weekly injection, versus 2.4 mg for standard Wegovy
    • 20.7% mean weight loss at 72 weeks in the Phase 3b STEP UP trial (n=1,407), versus 17.5% with the 2.4 mg dose and 2.4% with placebo
    • 33% of patients on the 7.2 mg dose achieved at least 25% weight loss
    • Wegovy HD is approved for adults with obesity who have tolerated the 2.4 mg dose for at least 4 weeks, alongside diet and exercise

    Why it matters

Obesity medicine is the most contested growth market in pharma, and dose escalation is how Novo Nordisk is defending its turf against Eli Lilly’s rival franchise. A 20.7% mean weight loss closes much of the efficacy gap that Lilly’s products had opened — and the once-weekly injectable now sits alongside Novo’s expanding Wegovy line, which already includes a pill formulation. For investors, a nationwide launch of a higher-priced, higher-margin formulation is a direct revenue and margin lever heading into 2027.

    What to watch

Uptake velocity is the first tell: watch whether payers cover the HD dose without step therapy, since insurers have been tightening GLP-1 coverage. Next, Lilly’s pipeline response — its next-generation obesity assets are the other side of this arms race. Finally, watch Novo’s fourth-quarter prescription data for signs that HD is pulling patients up from the 2.4 mg dose rather than simply splitting the existing base.
